Overview

Only caring for ones physical health concerns is simply not enough. To put individuals and families on a path towards good health, stability, and economic independence, we must look at every part of someone's life and offer the appropriate combination of healthcare, education, and social support. With this in mind, Mary's Center developed their Social Change Model (SCM), which offers medical, dental, and behavioral health services for the entire family, along with social services and family literacy services – all under one roof.

In Washington, Dc, 80% of health outcomes are determined by social, structural, and physical determinants of health as well as individual behaviors. Residents of low-income areas of Washington, DC consistently have worse
health outcomes and less access to health care than those who live in more
affluent areas of the city.
